- Once submitted your referral will be reviewed by the Care Team, and an initial acknowledgement will be made within
7 days (via email or phone call) - We will then contact the family to advise of acceptance, and what happens next. Or, in the case that the referral has been declined, the reasons for that.
- If the referral is urgent, as end of life care support is required – please phone our Care Team on 0117 9582172 once you have made the referral to highlight the request so that we can action urgently.
